I've had between none and two. Third isn't permaban, it's just temporary until they expire (there's more info on the infractions page somewhere).

For the WoW Forums, I've been suspended twice, once for a day (excessive spacing) and once for a week (semi-pornographic content), so I don't post there any more, because their suspensions don't expire.

You don't. That's the purpose of permanent bans.

Also, it's pretty hard to get permanently banned on these forums so you're safe.
