    SnD doesn't have a cooldown?

    Seems like ForteXorcist to me (as you can see hemorrhage/deadly poison ticks on the bar), and it's located just below the kittens representing comobpoints.

    You can do that with about a thousand addons. The most popular ones for that kind of tracking are Weak Auras and TellMeWhen.
