If your gear isn't very good at level 90 (I'm talking a fres 90 with awful gear) and you don't know the fights or where to go, it may be a little tricky. Once you go through it though, the difficulty drops significantly the second time around.

Reliquary of Souls is the only tough one, with the damage reflection and the disorient you need to interupt (or have a pet to burn through it). Everything else you can pretty much burn through mechanics or figure out along the way.
