Optimal use of Black Blood of Y'Shaarj?

Howdy. Does anyone know what the optimal use of this trinket proc is? Assuming running standard Insanity+DI spec, I've played with the following scenarios:

1. Pop plague at any ammount of orbs as soon as proc starts and start channeling Insanity, refreshing each second for new Int stack, then finishing with dot refresh at exactly 10 stacks.
2. Same as above, but refeshing dots at 9 stacks and channeling one last Insanity at 10 stacks
3. Keep orbs if below 3, continue normal rotation and only refresh dots at 10 stacks

But can't for the life of mine figure out the best application of the proc. Does anyone know the best use for it?

This is the guide I used and it was pretty easy to track. I used a WA that would track stacks nice and big right in my screen which would allow me to make the right choice
