Balance trinkets as of 6.2.3?

Yes my guild got to 13/13 heroic a month ago and has only killed mythic assault since then. Our latest heroic Iron Reaver and Kormrok kills were 3:31 and 3:18 respectively. I'm guessing that is not what is meant by "very short fights", especially since hReaver is still going into air phase for us.

The problem with IG is that it has a very low uptime (15 second on use effect with a 1.5 cd making it's uptime 17% if used on CD), so you need really short fights to have a decent uptime (less than 1 minute). It's not useless of course and it'll do a decent job regardless of kill-time, but it's significantly worse than DSI for example and you're better off using OoV over it for normal length fights (such as the ones you mentioned).

no*, and no.

*(this assumes you have 2 better options for the trinket slots - obviously if you have only 1 good trinket, and nothing else, POF can fill your second slot. But remember, Seed, DSI, OoV are all better than POF)
