Well, at the very least I couldn't reverse image search it, so it seems to be new. I would argue it might be Halloween art but the male/female seems to fit.

And if it is an expansion rather than Diablo 4 as so many people have been claiming, I CALLED THAT SUCKAH!

Think it was already thought to be a second expansion: WoWHeaded explaiend it a bit involving a 'Diablo 3: Realm of Evils' Shirt and poster they found on the store.
