Would say Player fault, however Revival should be reworked. Either it limits design space or completely useless at times.

no matter who's fault it was, they seem to take it in good humor though. I know of my raiding experience in the past I have had raid leaders that would flip thier shit and then some if that happened.

Exact same thing happened in my guild. Raidleader called for Revival just for the healing in P3 of Botanist, immediately regretted it when it got used and the tank got dispelled, apologized for the call and we tried again. It's pretty annoying(and the dispel has very limited use outside of trash, usually), but can be worked around.
