

They are private companies yes, but as others have stated there are a whole shit loads of laws and regulations that prevent hospitals form just checkmarking boxes for more money.
I've been taking classes in getting Certifications for EHR and CMAA, which is Medically Administrative Assistants. Putting in false codes, aka upcoding for services not provided to get more money is a sure fire way to open yourself up to getting sued. Not just audited by the OIG. The Administrator of the office isn't responsible for doing the actual coding. the Billing and Coders are for big hospitals. So the Admin can't just sneak a code in very easily as those people see the forms repeatedly.
Its a bureaucracy of rules and regulations where medical codes are checked and rechecked over and over.

Then a year down the road my Boss suddenly gets investigated for Fraud and Abuse and eventually charged, the facility gets closed down and I'm out of a job.
I look for a new job in the same profession, but everyone has heard about how I was complicit in the illegal activity my boss was doing. And suddenly I'm not as hire-able because I was ethically compromised and I can't be trusted to look out for the well-being of the patient and their best interests and could very well get my new boss in deep shit because of it.

I work for a non-profit teaching hospital. I work intimately with every aspect of this hospital. I see what billing, doctors, even the CEO do. I’m confident they are honest, especially since the government watches us like a hawk. There are SO many laws and regulations that any oddity is scrutinized and investigated.
We lost some Medicare money because the hospital, while one of the best in the country, takes the most difficult cases, which causes our morbidity to look a little high.
Hell the scenario you’re describing is exactly the type of thing that is drilled into us in a yearly basis: false billing is a huge deal. Any smell of that gets multiple people fired. In fact, it’s the most strict thing looked at for non-doctors other than straight up HIPAA/privacy violations.

In non-hospital jobs, I’ve told my boss that I will not do certain unethical things, and brought it to the attention of his superiors - as far back as my high school job. He's not an appointee, and was the acting IG, but was still the acting IG. The DOT disputes he was ever the IG in any capacity, which is a bit or a problem because his own bio on the OIG website lists him (still) as the Acting Inspector General, and the OIG's press release announcing Behm's inclusion in the pandemic response team lists him as Acting IG for DOT.
He was also, conveniently, apparently investigating possible conflicts of interest between Secretary of Transportation Elain Chao and her husband, Mitch McConnell.
