Addon name that shows active talent with equipped trinket/legendary

Hello everyone,

I have been wondering for a while about the name of the addon that shows the talent used, legendaries and trinket equipped in many streamers' UI.
It shows the icon and name of each talent actively used with the legendary and trinket equipped for me.

I asked in multiple chats and all they spammed was !ui or !addons and then after going through the list of addons, I could not find the one that I was looking for.

Since the twitch chat is as usual useless, I am asking on this forum for help.
