Yes rank 1 and 2 drop in LFR .Not sure where rank 3 drops , can find no one that has got it . I have cleared lfr 2 times did most bosses on norm 2 times and no rank 3 .

I got rank 1 and 2 in hc runs. Pretty sure its just random.

Our raid leader has rank 3 and he had it drop from a mythic boss kill. Obviously this doesn't prove it's mythic only and I would be highly surprised if it was, but it's proof it exists.
I don't know the recipe for success, but I know that the recipe for failure is trying to please everyone.

Guildie got rank 3 on mythic 2nd boss, but again small sample size.
