In the BFA opening cinematic she's beautiful but her teeth are riddled with plaque.

Not really. Ugly characters are too popular. Just look at Vol'jin.

He was so popular that he went from being a garrison prop to dead in 1 expansion.

If I ever invented a time machine the first thing I'd do is go back in time and force Blizzard at nuke-point to design Sylvanas as an ugly, rotting corpse. I know rule of cool dictates she has to look HOT AND SEXY tm like every other character (male or female) and no one is allowed to be ugly because that's not COOOOOL, but a big part of Sylvanas' character is the fact she lost everything from her living life, including (as we're told over and over but never actually SHOWN) her beauty. She doesn't even look like she's dead, she looks like a 'dark elf' or something. Show a picture of Sylvanas to someone who doesn't know anything about Warcraft lore and ask them what she is; literally no one will guess she's undead.

Anyways, back more on topic, she's at least looking better each cinematic. Blizzard has really had a hard time making humans or human-like characters look right, starting with Varian in the Legion cinematic. The troll guy, for instance, looks goofier than fuck with his uncanny valley human face. It's obvious Blizzard is struggling to make non-orcs look right. Sylvanas changes every single cinematic, and while she looks a little weird, you can see how they're getting more comfortable with motion capturing the basic human form.
