I think the obvious lesson here is that if there was a political party who portrayed evil CEOs and corporations as villains instead of blaming powerless immigrants it would be like shooting fish in a barrel for votes. It's too bad we don't have such a left wing party that would weave such an obvious narrative based on something everyone knows is true.

Moreover, the response from these insurance companies isn’t going to be one of “hmm maybe we should be less exploitive dickheads,” it’s going to be to turtle up and claim that suddenly they’re the victims under attack.

The life insurance claim was denied. The shot did not have prior authorization and was performed by an out of network provider.
EDIT: I say that as someone who works in a doc's office. We learned what words to put in the prior authorizations to avoid the automatic denial. Which is appealed, and responded to in 7 business days. Then you submit MORE paperwork for the appeal (which may include paperwork sent initially or in the appeal), which will be responded to within 7 business days.
At any point they may not receive the fax. Because reasons.
My office manager would not let me set it up to fax the patient's records (20-60 pages) at 9 PM, and resend it every 15 minutes until 8AM. Just to make sure they receive it.

Really, it seems to boil down to whether you can make enough of a profit off your serial killing, for shareholders in your serial-murder business.

Anecdotal personal experience. My wife was hospitalized twice in June this year. She ended up having to be intubated for 4 days the second time. She is on Medicare/Medigap. Everything, from nurses, doctors, facilities, room, etc., were excellent. Even the aftercare was superb. The doctor wanted her to be on oxygen at home. She was released in the morning, and in afternoon a technician came to our home to set up the Oxygen machine (no need to change tanks) with extended 50-foot hose which allowed my wife to move around freely. The same with the breathing therapist. She called us. Set up the schedule and took care of everything. I think I paid out of pocket around $900 in total.
