It's not extremely difficult at all, the reason people are suspect at all instead of immediately accepting it as real is because of how not difficult it is.
It is still a great deal of effort to build and plan, and design, much more than a text leak or AI image, but it's not remotely impossible.
We've literally seen staged "in-game" fakes before, from the lead into BfA's reveal to even it's end.
I gotta mention, if the images are real, it's strange that they aren't basing the titan architecture on anything new but just keep going with the Tyrhold architecture.
We usually either receive a brand new take on titan tech, like Uldum, Uldir and Mogu'shan or they are updating and reiterating on the old Uldaman aesthetic, like in Ulduar and Tyrhold.
Though the latter are each several expansions apart.
Even in legion, Halls of Valor looks completely unique.

Depending on the showcase video i guess, this could be just one part of itStaging the fight as a background detail is both interesting from a hiding detail point of view, and also would blizzard stage a fight like that so far in the background with a brand new race
the thing about those images is that they don't show anything interesting, only a bunch of reused assets. usually one wants to show the best side of something they're trying to sell, but idk i might be wrong.
Ok idk how I got fooled by this but thanks to the one posting the briarhorn pet in closer details. I can safely tell its fake, the adult briarhorn just have some very old and crusty textures (MOP) it would be unlikely to be their work especially when you see how smooth are the creature models nowadays
Compare new dragons models with olds one etc. Its way too much crusty to be wow current artstyle. Also I feel like that aerie peak statue might just be the mount the gryfon stone mount from the anniversary rigged that way and with the glow deleted through .blp on photoshop
If I had access to a sandbox version of Dragonflight and some kind of world edit software, given enough time (like a few days), I could probably replicate the environments shown in the images since they seem to be mostly showing reused assets. People do these kinds of things a lot on servers-which-shall-not-be-named.
Creating the dwarf and dino models is the actual hard part and not something I would be able to do since you need to actually know how to make custom models. At best I would be able to paint over basic dwarf and dino models in Photoshop to make them look like they're custom models, but people would probably be able to tell.
TLDR: Pretty damn hard, but not impossible for someone with the right tools and skillset.
Everyone can come to their own conclusion, but the hands on both frozen dwarf and the other ogre looking something dwarf theres something weird about the hands, one looks better than the other, that one thing is sus to me.

Those titan pillars in the Dino pic look like they have been added later into the pic. Probably after someone posted that photo of the Blizzcon set pieces.
