I can't recall N'Zoth ever mentioning anything about the "Void Lords".
What if the Old Gods genuinely don't know they are puppets and instead think they're in control?
Do the Old Gods have free-will or are they basically mind-controlled by the Void Lords?

Well the staff from the latest Trading Post reward very clearly states that Dimensius gave it as a reward, seemingly for the Black Empire flourishing, so there is that.

Quelthalas probably will only get new zones in the form of expanded Amani Woods and void portal areas.
Northrend is weird because we don't know of any areas nearby it, but they will probably use areas introduced in War of the Scaleborn (like Harrowsdeep under ICC for the Iridikron raid)

Just for specifics sake, it's not like -only- Ulduar was mentioned. We were told we'd go to the winterly "lands" plural.

What's the difference in redoing an old zone & creating an entirely new one? (Nothing. there is no difference.) They've been literally saying they wanted to redo Silvermoon longer than they've said they wanted to remake the old world. The implication is that we've only seen a few bits of the entirety of Quel'thalas. It's an island nation. Metzen literally used the word "islands" in his summary of Midnight.
Metzen quote can mean either option, really. We wont know for sure for years.
Sometimes, the light of the moon is a key to other spaces. I've found a place where, for a night or two, the streets curve in unfamiliar ways. If I walk here, I might find insight, or I might be touched by madness.
