personally, the most fail dungeon is stockades. no reason to go there besides a couple quests, there isn't even any good loot.

And as for BRD/Stockades.. best XP there is imo. Level 46 Hunter rested + XP shoulders, cleared a single BRD and came out at 50 and a nice shiny purple chest for when I ding 52. :P
As for dungeons that fail.. anything that contained a boss or fight that depended on a single player to do something right at a specific time. My personal favourite in this case was always Shadow Labs in a pug. Firstly the QQ people had to listen too on wipes with the Voidwalker boss before tanks learned to simply move from them and not blame the poor dps guy running about trying to kill em on spawns and then if that didn't make the group cry watching the Tank fail badly and running in and out on Murmur.. (mostly thinking from a Heroic pov also.)
My favourite to play tho was all/most TBC heroics when they first were being runned.. was a great feeling having a newly dinged 70 and rushing into instances with your rep/key ready to be one shot by any mispull. Having the <luck/skill/booster/whatever you wanna class it as> to be able to farm most of them in one day while randoms struggle to maybe clear one in which you have cleared 3/4 was great.

All WotLK instances. Those are only zerging and go out after. In TBC, you needed strategies and CC to clear them.

Vanilla: Gnomeregan, i just hate this instance since the moment i set one foot in there. Only thing compareble worse is DireMaul(every wing...)
BC: SteamVaults, I was in there waytoomany times, i could tank the entire instance with closed eyes
WotLK: Gun'Drak, because of the snake boss, and much more things in there....
Raid:
Vanilla: Naxx..... I never got the Attunement for Tier3 because i never raided on the Day, we went to Patchwerk. The Bossfights inthere are all awsome, but it was waaay too difficult for us.
BC: Didn't raid much in BC. But never liked Karazhan. beside the Aran fight. Vashji was kind of fun for me, and Kael'Thas was all about Legendarys xD
WotLK: Malygos, because the drakes don't scale with the gear you've got... Or after that the screwed up Naxx, which was way easier than in Vanilla
for me its wailing caverns....always get lost in that damn place

the closest I come to hating something is tanking the nexus belf wing on my protadin, it sucks

My mosted hated classic dungeon was BFD. This dungeon had nothing going for it. In addition to being in the middle of nowhere at a point that was a mile away from any flypoint it had:
Nagas
Murlocs
Those crawdad guys
Gnomes (the twilight guys)
This makes it by far the most evil instance in the game.
Classic raid would be AQ40 because of the twin emps. I had quite a few 1% wipes on that boss at lvl 60, and I've been in there and wiped with 70s and 80s on twin emps because people can't understand the strategy. Hurrah for Twin Valks in the next patch!
rfd fails... takes too long and if someone hasnt done the escort you can add another 10 min to the run..
for BC, i really really hated sethekk halls with people never knowing what to do on the final boss, so many wipes
for wrath although i know how to run the oculus, i hate having to teach most of the group how to run the damn place everytime i go, no one really taught me (which is why i took a lot of damage from the 2nd boss lol) but i learned
i started playing wow during BC so i cant judge old-school raids
for BC raids, the i really liked the raids, though in certain bosses people were just idiots and made us wipe over and over (void reaver, somehow people couldnt move, shade of aran, people loved to move on that fight, prince (usually dependent on location of infernos) and clicking the boxes in mag's)
in wrath i have really touched the raids much either
Dungeons I hate (because I am horde):
Dis-liked Uldaman - About a 30 minute travel time on mount or an hour long travel time on foot depending where your running from and whats in your way.
Dis-liked Gnomer - Pretty much the same as Uldaman. Travel time = not worth it.
Dis-liked Wailing Caverns - It's a design mess. If you don't know what your doing or are new to the game you will get lost.
Over all I enjoy many of the instances and have since the only raid's open at the time was MC and BWL was being released.
I found as I leveled more characters I would skip dungeons because the xp gain compared to questing was not worth it sometimes.
Now I have an 80 DK who can solo most of the level 60 raid content and some of the level 70 raid content.
ZG - Extremely easy; I think the hardest boss is Bat boss because she can heal. And if I am CC'd by her I cant interupt it. Also learned the hard way with Hakkar haha I was bored at about 45% and figured I would pop army of the dead and finish him off and he reset cause I got MC'd.
Onyxia - She's a joke.
MC - Extremely easy except for Magmadar Trash and the boss that has 4 healers.
BWL - Pretty easy once you get an ony cloak and can figure out the first boss. MC and break 2-3 egg's. Release and kill and heal and then MC and break a few more egg's and rinse repeat.
AQ20 - Killed all boss's except for the flying boss and the ooze boss.
AQ40 - Have not attempted yet. I know I could not get past twin emp's so I never tried.
As far as level 70 instances:
All dungeons.
Karazhan - Cleared all up to Curator but I could not put my dot's on curator to heal so it was a wipe .
Blood Tanking Hybrid spec + 5/5 T8.5 Tanking gear = win.
Most fail dungeon ever will always be Maraudon to me.
Seriously, running that place and with your hearth is down? You might as well wait it out instead of running back.
Maraudon can DIAF.

Occulus and Maly drakes weren't hard. Or bugged. The only errors with the drakes is one that's occuring somewhere between the chair and the keyboard.
Before you argue "I PICKED CLASS TO PLAY CLASS NOT DRAGON," it's five minutes a day/week TOPS. QQ more. I guess you're going to say Teron Gorefiend and Kil'jaeden were terrible boss designs because part of the mechanic was playing as something that wasn't your class?
Worst zone ever: Mount Hyjal.
