Should be just like a normal one:
/cast [@mouseover,help,nodead] Leap of Faith

Doesn't seem to be working for me that way.
The standard UI doesn't recognize other mouse button inputs on unit frames besides mouse 1 and mouse 2.

The best you can do is to create a standard mouseover macro, go to keybindings, and make whatever slot your macro is in the button you wish to use.
Hmm my mouse over wind shear is bound to button 5 and it works perfectly for me.
Mouse key bindings do not work while clicking a mouse-interactive frame (f.e. unit frames). They will work when clicking units in the game world.
The only solution is to use an addon that modifies your unit frames' secure attributes such as Clique.
