I think the people that say a player means to hit someone helmet to helmet outside of situations where the player is stood up and another launches themselves crown 1st have ever actually played. If you lower yourself to make a tackle shoulder first, and the other person lowers after (happens constantly with QBs being sacked/sliding or WRs bracing for hits) you can't avoid it. You see guys get flagged for late hits on sliding QBs all the time despite trying to avoid the QB and just clipping them purely from momentum for example. The only way to make sure you don't hit someone's head is to tackle low, in which case you get people complaining about knee hunting. There is no way to always tackle in a way that looks 100% clean.
