When we finish the game in one level and start the next level (e.g.: end Normal and starts Nightmare) do we keep our items and skills or we have to start all over?

You wouldnt kill the first enemy on Nightmare if you had to start all over again ^^

You keep everything you had when you beat normal. You will just start from the beginning of the story, with all your items and skills, but on a hard difficulty.

From there you continue to level up and get better gear than you could get in normal.
