I'm not sure what you mean, I think she was Alter in London just because that's how she was summoned. They did give a reason for her increase in chest size, though (magical spear gives magical gifts lol). Maybe you got confused with that?

This version of her is because Bedivere didn't throw Excalibur in the lake like he was supposed to, so Arturia didn't die. She ended up ascending because of the spear and became a Divine Spirit. She's barely even Arturia anymore, she's basically just an avatar of the spear and can't exist without it.

Ye, idk. I haven't really followed much of the lore (mostly because I can't read moonrunes and I haven't bothered to see if there's a translated version yet), I just try and roll my favorite Servants/grills and call it a day.
