Paladin Tier 16 helmet
The chest, shoulders and legs look to be from the Truesteel Battleplate set.
Belt #1 OR #2
Gloves #1 OR #2
Tabard is the Stormwind Tabard from the Dwarven Foundry.
Not sure on the boots but I suppose those from the Trusteel Battleplate would fit well.
The weapon is Admiral Taylor's Greatsword.

But Blizzards in their "infinite wistdom", removed its source from the game because they could not be bothered with retuning the MoP CM after the stats change on patch 6.0
