Well I was once in a group for Shrine of the Storm that wiped repeatedly on first trash pack. Tank didn't use mitigation properly, there was no cc on mobs and every single heal the mobs cast went off (no kicks). It was teeming and it was a pack with 2 healers and the shrine templar that makes allies take 75% less damage. Despite me saying otherwise the tank repeatedly yelled to ignore the templar and 'focus the healers'. I left.
I usually do M+ with guildies so obviously I'm not gonna leave their runs. If we do runs in time (which I obviously strive to do) then great, but if we don't it's not the end of the world, it's still better to complete the run (if possible) than leave it. You've already invested X amount of time, after all.
If I'm in a PuG (which rarely happens but it's sometimes the case) I'll leave if I'm receiving abuse (this has never happened, but I know I would leave if it did even if it was my own key) or if we kept wiping at the same point for a long period of time. Like, 1 hour wiping at the same point, whether it's a boss or particularly difficult trash pack, whatever. Should point out that I'm always happy to receive criticism as it may help me improve, but there's a difference between criticism and abuse.
I'm speaking hypothetically though as luckily this has never happened to me, but everyone has their limits and you can only spend so much time on 1 Mythic key.
It also depends on the group - if your group's goal was to do the key in time but it turns out the key will be depleted, my personal view is that you should just stay and complete it as at least you might get something out of it still. But some people might leave and it's partly why I don't bother with PuGs. At least if I do M+ with guildmates I can communicate with them on Discord and worst case scenario, we fail but we still have a bit of fun laughing and joking with each other.
More of a raider than a M+ person though, honestly I just do my weekly cache. Don't have the free time to invest in grinding M+ anyway.

A rogue we invited for +13 TD left after one wipe on third boss and called us useless. He was the first to die.
We always do M+ with a couple of friends, usually inviting 1-2 randoms, so overall, I have no reason to leave myself.
In all my time doing M+ i have only ever left 1 dungeon.
Somehow the keyholder had got himself a +19 key back in legion, he was maybe geared for a 15, but his skill was around +8 level. He just couldn't cope and he was the healer. As a DH tank back in legion I was fine, but the rest of the group was dying constantly to just group damage. He had no idea how to play the class at all.
It was an EoA, we had got to the first boss after about 3 wipes on trash and we wiped 5 more times on first boss. I was giving him pointers and as much help as I could but he just wasn't getting it. We had been at it for about 45 minutes at this point I think when I called it a day,
I generally wont leave if its just going to be a long run but when it is obvious the group will not be able to complete the dungeon thats when I will leave. As others have said, I would also leave for toxic behaviour but I have never come across that myself really, few flamers because I've done something slightly differently to them, but when it works out fine they usually just shut up.
I don't leave groups until the run is over. I'm the one who'll even try to talk down toxic people if they try acting up. Oddly, I've never been kicked either.
Toxic people is cause #1.
If we're continually wiping on something (5+ wipes) and it seems clear to me that we aren't going to clear it due to people being bad at DPS/Healing/Whatever, I'll also be likely to leave, though it isn't like I'm ready with the trigger when we're on the 5th wipe.
Generally, since I've stopped raiding, I don't really need gear for any particular reason. So if anyone starts being toxic in a run, I'll consider leaving. And if they either continue, or there's a 2nd instance of it, I will leave. I don't need the gear, and I don't care about your hissy-fit or key (Or mine, for that matter). I'm out. There's usually a part of me that'll think about how they'll learn not to be toxic eventually if this keeps happening, but realistically, that won't happen.
I only leave when it's obvious the group just can't finish the dungeon.$
But that can be spotted early in. Only left a single MM+ this expansion. Veting your group is mandatory, not an option. Both when you make the group AND when you join a group.
REAL toxic behavior, me as tank doing more than 50% of total dmg on trash and bosses, ppl being generally shit for the content, if i join an +2 for lols its fine, if its an +9 and im still doing 60% of all dmg im gon peace the fuck out.
Dont play atm, during Legion i did A LOT of M+, i left 3 that i can remember, outside of the ones were 1-2 ppl DCed and never came back and we couldt 3-4man it.

i can't recall a case in which i was the first to leave, even when the run was a catastrophic failure with like ~150 deaths
Toxic behavior is my quickest way out of an M+.
Doesn't apply here but in most LFG runs, if someone is a big enough jerk and threatens to kick anyone because they aren't up to their level of good, I start a vote on kicking them. You'd be amazed how often it works.
I generally run with guildies/friends - we have mutually decided to abandon keys when it was obvious we were never going to be able to complete one particular dungeon with certain affixes. Otherwise, we'll complete them anyway even if we know we won't make the timer and use it as a learning experience for next time.
As the whole point of mythic plus is to push and push until you reach a gear/skill barrier, there are ALWAYS going to be unsuccessful attempts at higher keys if you keep pushing. Not making the timer is definitely not a reason to leave but rudeness/toxicity definitely is.
Someone flaming, ninjapulling, fucking up several times on mechanics, tank taking too long to pull, low dps, tanks not using any mitigation.

A ret paladin, the leader and key holder, who has not been interrupting / stunning / or swapping to orbs.
We wipe 3 times on first boss because we had 3 melee and they would get rekt by every avoidable damage you can take while boss only chases after healer .... because 3 melee...
He proceeds to call us a shitshow and asks if we know what to do.
Yeah, we clearly need to learn. I'll go join some lower keys and learn. I left. I do feel bad for the other 3 but ... sigh

I only leave for two reasons: toxicity and players who are outright bad/won't stop making the same mistakes.
I tank M+, and whether people start calling out others in toxic manners or berated me for not following their strats, or trying to blame every wipe on me, I'm out. Today I was tanking a Siege 8 on my DH alt, we wipe on the trash packs right before the first boss's room because the frost mage couldn't control their frost orb and ended up overpulling. First thing in chat was "tank you can't pull that much". We then wiped 3 times on the first boss because the group repeadetly ignored my instructions to leave the 2nd Vanguard add CC'd, and only target marksmen/the boss. The healer also died each pull due to not running the boss to bombs. The group told me I need to learn how to tank.
If people make mistakes and cause a few wipes, its fine. It happens to everyone. But if players constantly fuck up the same shit or are just bad at the game/undergeared, I'm out.
I don't care about other people's keys. I am investing my time into this game as well, and if I feel like a run is a waste, cya.

People saying they know tactics before I pull a boss, only to wipe to the very first ability that requires anything from them. After asking again if they know a boss and stay quiet, they can enjoy being quiet with 1 less person in the run.
Depends on the day, however its usually a combination of stuff.
