Corrupted Gear - Negative Effects

If it has a negative effect such as occasionally causes damage to you, or spawns a creature that follows you.

Does that only proc in Combat, or can I be hanging out in a major city, and randomly get in combat due to the corrupted gear?

Negative effects activate when you enter combat afaik. I don't know what happens with them once you leave combat, though.
