Missing reward from ToC 10 heroic.

My guild and I just killed off anub as usual in trial of the crusader 10 man heroic with exactly 47 attempts left, we haven't made insanity yet but mad skill quite a few times, I am confused tho shouldn't the cloaks dropping be a 100% chance of reward? because we received x4 Tokens, a crusader orb and the two-handed axe Lothar's Edge but am I mistaken or what because I believe when doing mad skill ach with 47 attempts left we should get a cloak?
Sorry if I am mistaken and feel free to flame me if that is the case.

You only get cloaks with a successful Tribute to Insanity.

Ye, Cloaks are 50 attemps remaining(+mount), 45 is just moar trophys, 25+ is weapon.

1-24: Two trophies.
25-44: Two trophies and a weapon.
45-49: Four trophies and a weapon.
50: Four trophies, a weapon, a mount and a cloak.

The 25 man is the same except that the mount is only from immortality.
