Heh, I was just thinking of making a thread like this after going back and watching a bunch of anime openings/endings sparked by the fantastic Madoka ending.
Anyways, my favorite is probably still the 5th One Piece opening. It just had a great feel to it and really set up that entire arc well. The fact that the Water7/Enies Lobby arc is my favorite helps too.
Couple other really good openings:
And for some reason, the only anime opening that I just have to watch all the way through, every time. Just to catchy of a song plus Aya's dance near the end.

People have already named my favorites of Samurai Champloo and Cowboy Bebop. I also thought Claymore's intro was decent, but not on the level of the previously named series.

Ah yes, Bakemonogatari had some great music. The ending theme is still probably my favorite.
