If you're doing NV consider the Project Nevada mod. It does take an external program (well I think you can get around it) but it's not that hard to set up. It can be set up in ways to break the game, but in other ways it's incredibly useful and opens up the game for more options. You can turn on or off specific mods or parts within mods so if you see one thing inside a suite that you don't want, don't worry: it can be changed likely in game.

A lot of the stuff in Project Nevada is so basic in other shooters or of the nature that you read it and think "Wow, I can't believe the game doesn't have that!"
I personally use almost everything from Project Nevada in my game, except for the Cyberware module. I wasn't too big into the implant idea when I was playing the game-proper, and adding a whole gear up via implants idea didn't appeal to me. That and I raise the level cap and disable slower leveling.
Also consider a World of Pain:

Adds around 100+ extra locations, some are tiny like gas stations, some are big underground sewers and whatnot. It does have dialogue for all the extra quests it adds, but I don't think it's voiced. The big downside for this one and me is that 1) it can be very distracting in a new game and 2) It's a lot of potential extra loot. That can really offset a game as much as knocking over The Silver Rush can. ^_^
