Inside it's fine. What's the trick to make it show all people in world?

Under the Layout tab, and in the Layouts section, set "40 Player Raid Layout" to "By Group 40" (or the option including pets) and then check off the option right next to that pull-down menu entitled "World Raid as 40 Player." That should use the 40 player layout whenever you're in a raid group but outside an instance.
