Genuinely paranoid people usually don't think they're very paranoid.

If you know that your feeling is just paranoia and not based in reality, you're not very paranoid. This is why you don't evaluate someones mental health by asking "are you mentally healthy?".

I never search for my adult material, I only follow saved links and use tag lists to search, for fear that someone is watching. Same goes for embarrassing questions I need to ask the internet, I never type it out, I search for the thing involved and read dozens of pages about it, hoping to answer my question without asking it. I never think badly about people, for fear that they can read minds, and I never look at women, for fear they will find me ungentlemanly. (and because Asperger's makes recoiling from eye-contact a natural reflex.) Sometimes I auto-surrender when I play Hearthstone against a thief-priest, for fear they will make fun of the cards in my deck if they steal only the bad ones. Despite being a 100 MPH+ speed demon on back roads, I never even touch the speed limit on highways, since I might see a cop in a bad mood and get a ticket. Whenever I use someone else's Netflix, I only watch movies that it seems like they would watch, so they don't get any ideas from the suggestions that would come from what I want to watch. I've been writing a book since Junior High (I'm 23 now...) that will never get finished, partly because of paranoia that no one will like it AND because I throw everything away when I learn someone might come to my room and somehow read the unfinished and unedited draft.
