Have to disagree with the Angel mentions somewhat, and for one main reason; Connor, especially the awful Cordelia storyline.
It stepped back up when he was 'written out' of the series later on but he was such a crappy character.
In general though:
Blackadder
The Wire (S2 and S5 have good and bad moments, overall the series stays steady though)
Buffy the Vampire Slayer (dips at parts in S5 and S6)
Breaking Bad
Farscape only gets better (with rare exceptions)
Speciation Is Gradual
I can't think of a single one other than classics (and very short-lived shows) like Fawlty Towers. Even the very best shows curve down here and there or for entire seasons while still remaining great. I'd say the most consistent shows are usually lower quality shows like Modern Family that really don't have to do much to stay good.
I'd mention Stranger Things as one of the great shows that has stayed consistent in quality so far. Most of its cast were children that are now hitting puberty and adolescence at different rates, and I have a lot of admiration for how they handled that so successfully. It must be a minefield.
For anime Id say Cowboy Bebop, Lupin the Third (which started back in the 70s and is still going), and Trigun.
A year or two ago I would have said Dr. Who but the current doctor and series is TRASH.
Aqua Teen Hunger Force was always great I will never understand why CN canceled them when everyone from the actors to the writers wanted to keep on and the ratings were still great. It even had a successful movie that made 5.5 mill on a 750 thousand dollar budget.
Squidbillies is another gem that was great but canned for no reason.

Breaking Bad is the first and probably the only one that comes to mind. So many other series have on and off seasons, but BB got better all the way through.
Maybe to you, a lot of people including myself disagree. BB is known by the majority to have gotten better as it progressed.
