Hello, does anyone know what the name is on the addon that shows his Rend / CS in the middle of the screen and showing his potion buff/trinket proc in the middle right??

TellMeWhen is certainly one of the simpler addons for an icon based display and one I would recommend should such a display be all you need.
For a similar featureset for bars then I would advise NeedToKnow.
PowerAuras and WeakAuras are capable of such icon displays and also more graphical displays, with WeakAuras supporting even more such as bars and progress textures, where the texture or fill of a graphic will reflect the state of a quantity.
However while those two are pretty much default suggestions much of the time, they are overkill for the example in the video.
