It's ElvUI, with a few modifications.

Shouldn't be too hard to re-make for yourself.
unitframes: Pitbull 4.0
Action bars: Dominos or Bartender
Kgpanels: for the frames around chat, action bars, omen etc. looks like caith borders, not sure since the pic is so blurry.
Timer above unitframe: classtimer
Minimap: probably Chinchilla
Battletext: mikscrollingbattletext
raidframe: Grid
Plug-in, infobar: Titanpanel
Chat: Probably chatter
