The headpiece is Clefthood which is a small quest chain that comes from Nazrell via enter the dark portal. If your really lucky you didn't do this quest while leveling in outland.

Isca, your transmog is fun, but it is unfortunately overshadowed by the AMAZINGNESS OF THAT TOOL. I'm bookmarking the hell out of that website.
