These are pretty predictable answers.
I noticed something in the second case of each scenario.
None of them brought up that if they involved another bystander, they would actively commit murder.
While they might be accused of that in the first case where turning the trolley onto the single person, "murder" was unavoidable there. It's the same with the first doctor case, someone was going to die no matter what your actions were.
Sure, you could be accused of murder because you refused to help the already dying ones, but it would not be the same.
