Yes, you need proper paperwork. A Tattoo is not proper paperwork.

Don't they turn away people with Tatoos for blood donation? So wouldnt organ donation be even more picky.

As long as you don't get in a car accident within twelve months after your tattooing, then hey, you're golden. It'll speed up the process! Otherwise, it'd just be an ironic waste of your organs.
I lol'd at this, best answer. You can tattoo anything you like on yourself, doesn't make it so. And if you were in a bad enough accident that your ID was torn up and or burned beyond recognition chances are your organs didn't do much better. I am pretty sure when they identify the body they would check the organ donors database and see if that person was one or not anyway.
let me actually give you a serious response:
it depends and is a matter of law. In Germany, becoming a donor is fairly easy. All you need is to express your opinion about it and write it on a sheet of paper. If this doesn't exist, an oral Expression to a relative is also enough. If neither exists, your relatives will be asked what's going to happen with you.
So a Tattoo does actually make you a donor, even better if you put your signature under it.
However, you totally don't need to. Just make sure that you tell enough People to become one, make a second donor card and put it in a place people know where it is.
With these steps you will almost surely become a donor after your death.
It also doesn't speed things up, because in order to become a donor you have to be identified and your organs have to be tested for various diseases, and in almost all cases the last step is the far more time consuming.
An organ donor databse doesn't exist in many countries (like Germany)I am pretty sure when they identify the body they would check the organ donors database and see if that person was one or not anyway.

The only good it might do is supporting or refuting your family's "would have been a donor" argument, although even there, many hospitals won't touch it for fear of being sued.
Thought experiment aside, if it is something a person cared enough about to permanently mark their body, they should just spend the nanosecond to check the box and get an updated DL.
Just say yes to being a donor when you get your license?
