I really enjoy Warhammer but don't think it is a fair to name it among video game lore. 40k exists primarily outside of video games as both boardgames/miniature and in various rule books, novels and comics.
Like what if I made a game based on Ulysses or Pale Fire and called it the greatest video game story? Those novels being the greatest written in the English language.

Though I like Warhammer a lot. So, I am not too concerned about it.
Thats kinda true, there have not been many games going deep into the lore or made the universe justice other then the dawn of war serie and they do a alright job with story telling.
I think that will change though when Creative Assemblys warhammer fantasy game comes out, hopefully all races will be playable and with a good story will bring in more people to the warhammer ip, we have not seen anything from it but if it's anything like warhammer with the total war gameplay and a good story it's going to be freaking amazing.
Other then Warhammer lore i like Mass Effect alot, Elderscrolls is great, Halo is alright, diablo and a few i cant think of now

warhammer (40k) and planescape. but as said probably not fair.
if you only take into consideration the lore presented in the game then planescape would still be one of the best. warhammer games are rather meeh in the story department imo.
mass effect and vampire bloodlines are also contenders. world of darkness is pretty extensive offline too though ...
40k, Witcher, Mass Effect, Dragon Age (overrated as it is), Starcraft, Warcraft.... Hmm.... Bioshock... And probably quite a few others honestly.

The main problem with almost all video game universes is that they mostly exist in books outside of the game(s) and not in the actual games.

People usually don't think of Metal Gear for its lore but it is very extensive.

While all these worlds are awesome. I jyust feel that they are so dangerous, i'd probably end up dying in either one. So for safety's sake I'd say the below

Ya, there are only a few games for each series, but all the games in each series exist in the same respective "universe" with varying levels of consistency and quality.

It freaking sad that FPSs nowadays can have better realised universes than WoW.

Darksiders were shaping up to have pretty interesting lore, but I'm not sure about future of the series...
Mass Effect was good but it's soooo fucked up with 3d game, it's beyond saving.
Mass Effect and KotOR had incredible lores. Well, Star Wars generally is incredible lore. But Mass Effect beats it, I would say. ;-)
Halo, Mass Effect, a lot of star wars games like Kotor and even TOR have crazy good lore
No Final Fantasy? The games may not be direct sequels but the lore is connected. And what about Zelda?

Not really... First of all WoW is ten years old, second of all some of the most praised videogames comes from FPS games such as Deus EX HR or Bioshock, where as other FPS games hardly have a story, like CS or CoD. Difference is that with Deus Ex and B-shock it's story > gameplay and with CoD/CS it's gameplay > story. It's all about definition and different strokes.
