Season 4... Just old dungeons and new ilvl?

As the title says. More or less of someone who did something practically unspeakable (swindling a incalculable amount of gold, deceiving raids for self-interest, etc) to the point it was so bad that the player could not escape his reputation as players made an active attempt to spread his name all across forums and other servers?

Quit the game? No. Faction change with a new name? Yes, actually. This guy was a notorious BG/Tol Barad honor farmer and always hid and collected honor. He more or less got blacklisted and faction changed.

Nope. Unfortunately. Some have server switched, but that was just a couple people.

Raid leader ninja'd Ashes of A'lar from me in early wrath. (Before people say master looter, shouldn't have trusted him, it wasn't a ninja, etc, he did state loot rules in raid chat beforehand and chose to ignore them when it actually dropped) He logged off. Hours later he logged back in with a different name.. unfortunately I never left the raid group and I saw the name. He was kicked from his guild and soon after server transferred. I checked up on his toon for a little while and it appeared that he quit playing soon after because all of his friends he had for 3+ years were on the server he left.

In addition to currently lacking a PC I have been also been a player who has unfortunately done something extremely stupid and have been lying low and anonymously for nine months now, spending my time watching TV and visiting this website. Someone I knew basically gave me a vast sum of gold (11k) in exchange for supporting his guild for a very long time to make up for the gold. I unintentionally betrayed that trust and faction servered with friends to play PVP and he sent me a crazy death threat so I got freaked out and am in this situation now.

This sounds like troll bait, given I recall you citing other reasons for not playing in other threads.
Though, if that's how you're going to be: why not just buy the gold from a gold farmer? 11k must be worth, what $1 these days? You roll a level 1, buy the gold, pay your friend back, and hopefully get caught/perm banned, move on and stop spamming the forums with troll bait. Everyone's happy.

This didn't happen in WoW, but I saw it in another MMO I played a few years ago.
On my server in FFXI, there was this guy that was pretty well-known for being an annoyingly bad player. He apparently got busted for distributing child pornography. He kept it hushed for a while, but some people that REALLY didn't like him did some serious internet sleuthing to pin the story on him. I can't remember the specifics, but it was something along the lines of people found the original story with his real name in it. They found an account on a bodybuilding forum that used that real name but used his FFXI name as his login, so it was pretty easy to tie it all to him. He denied the whole thing for a while, then took it to "I wasn't DISTRIBUTING child pornography, I was just in POSSESSION of some."
He left the game shortly after.
I wish I could remember the guy's name, the story and subsequent forum flamewars back in those days were pretty hilarious. I'm sure it wouldn't be hard to find via google, but I'm sleepy and I don't really want to search for anything regarding teh kiddie pronz.

Some player on my server actually faked his own death.
Forum thread, faked skype logs and staged calls from the loved ones. That shit was pretty crazy but after a few months someone discovered it was a fluke.

Had a guy take a guild member's character to raid on (he gave it to him when he quit), and then tried to sell it and lied about it to our raid leader, who was his friend IRL. Didn't go over well. The character ended up getting taken back and the guy was gkicked. I should mention he was playing that character because his old one got "hacked" and he couldn't get it back. After everyone realized he was trying to sell the other guy's character, we knew pretty quick what was actually going on.

Had a raid leader in BC / Wrath. Was as pretty decent RL in a decent higher than average mid-tier guild. Almost all the officers in the guild met up in Vegas once a year. All of them but him since he wasn't so into the stuff that went on there (drama for the most part). Finally after one of the trips he let them all know what he felt about them doing what equated to as bullshit (we usually lost 2-3 members every trip due to drama that went on DURING the trip). I smear campaign went back and forth for a while and after breaking the hearts of many members and fellow officers he quit the game.
I had him on battletag years ago. When RoS came out I noticed it pop up for the first time for years. Said hi, and we run some Torment. Good guy. Good intentions. Terribly executed and went to far.
