The name "Balls" works really well with the fishing achievement title "Salty".
And I once saw a druid that was called "Forestkin". That, to me, is the best name ever. Forest, Kin, both completely in theme with the druid. Yet only one letter away from foreskin.

I think you should consider making a Warlock or DK called "Taint"
My Fem Worgen DK is named Bichie.

i had a female undead hunter that i named rottentwat, had to name change so it became rottentwot, another name change, rottencoochie, in the process of all this i recieved 2 bans and was told that another offense would result in a permanent ban of my account so i named her threebans.
I had a druid named horseporn until I killed people at level 44 in the pirate cove in Tanaris during late TBC. Blizzard let me pick my name so I changed it to bearporn, that lasted all of 3 hours before they made a name for me.
Pretty sure they have better filters now, at least for non ascii names these days.
A lot can be done with combinations of titles and guild names. Such as the classic:

Maybe not creative, but you have to admire the dedication.
Had a buddy with the name 'Stinkydonkey' and it was flagged by someone and Blizzard forced a name change. Not sure exactly which rule he broke but it seems the name must have really upset someone enough for them to report it.
Needless to say, he wasn't that happy about it, so he's just going for obviously offensive names now.
An old guildie who has moved on to other games had a goblin mage named Murken.
He tried so hard to get the Salty title but wasn't ever able to get it.

I just rolled a new toon on a new server (just dinged 90!). He's an orc warrior named Jugsornot The Seeker and my wife used to have her rogue named "Analmutilate" which lasted for ~68 levels before the change. (She destroys plate-wearers in PVP so I imagine someone got butt-hurt after she killed them)
My husband knew a guy with the character name "Larryola" and got a forced name change, yet blizz has an npc named "master baiter" and a quest called "just the tip" Hmmm...
My mage was named Pines.
