This. Especially in games such as Skyrim or Fallout 3. For some reason I never managed to judge the distance at which it'd be safe to jump off of a ledge or building and then my character ends up dying spectacularly. In WoW my first memory is jumping off of Teldrassil and dying because for some strange reason I thought it'd be a good idea to jump off of a giant tree...

In MMOs or in single player games. I'm bad at judging distance. (After about 300-some fall deaths in WoW I got good at judging fall distance in Azeroth, though. :P)

If not self-inflicted dumbness, then the next runner up would be boss/gimmick fights that put me in a first-person shooting type of mode. Generally in space ships. I fail at being in first person.
