Based on what? She was never described as the most powerful mage. She's only described in the book as a powerful mage. Her being the most powerful human woman doesn't make her the most powerful of all.
Also, Khadgar is actually described as "one of the most powerful magi in Azerothian history." And "unrivaled in his wisdom and experience."

Would certainly be interesting and a nice change if this was a hint that Khadgar's wasn't all that he seems but I think Blizz was just lazy and decided to give him Atiesh as a nod to Vanilla and in order to make him seem more important and powerful.

They did clear it up. They spelled it out on both of the official WoD website and Khadgar's battle.net page.
He's also in Blastend Lands when the Iron Horde first started invading and is the one who goes to tell Varian what's happening.

Like many Vanilla legendaries, you had to do several raids to get it. So you did have to do AQ40.
Assembling the staff
Kel'Thuzad's underlings in Naxxramas each have a chance to drop a [Splinter of Atiesh].
Once all 40 Splinters have been found, they may be combined into the [Frame of Atiesh], which gives a quest to visit the dragon Anachronos. Players were then given a quest to obtain the staff head and base of Atiesh. They had to be at least neutral with the Brood of Nozdormu to be able to talk to Anachronos to complete the quest and hence be able to pick up the following quest to obtain the head and base.
Kel'Thuzad drops the [Staff Head of Atiesh].
The [Base of Atiesh] drops from C'Thun in the Temple of Ahn'Qiraj.
The final step was going to Stratholme to cleanse the staff, followed by slaying Atiesh. The summoning location was in the Festival Lane west of the Alonsus Chapel, in between the western ("Scarlet") and eastern (Scourge) districts of Stratholme.
There is no Medivh of Draenor's timeline. The worlds were never connected as Garrosh arrived years before the portal was constructed.
And if there were we have no reason to believe he would be dead, and there's no fucking way his staff could wind up in the hands of our Khadgar.
