The shoulders have a bit of a Cataclysm vibe for me... They had a few models in that expansion
that had that shape. I went through Icy Veins Tier set page but didn't find anything quite close.

It's not a set that exists in the game, and you're not going to be able to find anything to accurately reproduce it.
The best you might be able to do is Preist Tier 3 or 7.
