Help! Addon/Macro to swap gear when changing talents

Hey all, so what im looking for is a addon or a macro that changes my gear when changing into certain talents in the tree. For example, as a shadow priest when i swap from Auspicious spirits to Clarity of Power, i would like a way to automatically swap my gear.

Anyone?
