It says when target is below than 30% HP, bleeds and poisons deal 40% increased damage

Does it increase Agonizing Poison effectiveness or something like that?

It doesn't. Your bleeds/envenom are already dealing more damage because of agonizing poison so it's not like you're missing out on anything.
