Just the highest item level. In instanced PvP you get a stat template, so the stats you get are pre-determined.

Instanced pvp (BG, arena) you want highest ilvl. What stats it got dosnt matter.
Your ilvl then adds a few % more to the pvp templates (stat ratio you got) based on your ilvl.

What about trinkets? Are they working in instanced PVP?
Or only pvp trinkets work there?
What about gems? IF only ilvl matters.
