It's simple. Why does a guy who shouts anti-Islam slurs get labelled mentally ill, but someone shouting allah ackbar doesn't? They both kill people for ideological reasons. The reason it matters is slapping the mental illness label prevents discussion into factors that led him to this behavior, even if he is actually mentally ill.
