Differentiating bad healing from bad tanking

Is there an easy way to do this without explicitly asking what the other person thinks? I've only healed, and sometimes tanks will die really quick or just be hard to keep up and I can't tell if I'm forgetting/messing something up or if the tank is.

It seems like there's not much that could be messed up as a healer, but is it that way for tanks as well?

As a tank you can mess up too. If the tank isn't using active mitigation then he'll be considerably harder to heal.
As a paladin you need to keep shield of the righteous up, as a DH there's spikes, as a warrior there's ignore pain and so on and so on. There's more of course, but that's just off the top of my head.

Most of the time if the tank suddenly dies it's the tank's fault. If you can't keep him up while spamming heals, it's the tanks fault (or a special circumstance is at play)

If he doesn't receive heals for a few seconds and dies, its your fault
