Rarespawn tracking mod?

I know they used to exist but someone told me that they all broke when 4.0.3 hit. Does anybody know of a new (or fixed) rarespawn tracking mod?

Try using NPCScan.

Use NPCScan as the addon, but I am also sure that there is a bug with the rare mobs, and it was mentioned in PTR notes that they are bringing them back?

I've been using NPCScan like the others have suggested. If you already have it but haven't updated in a while, you'll want to do that. But mine works for Cataclysm rares. I've found a few in Twilight Highlands and Uldum already. I just wish they had an achievement for the rare hunters again.

Stockli, some of the Old World rares "disappeared" and stopped spawning in the 4.0.3a patch - the 4.0.6 patch (the patch notes on the front page) will be bringing back some of them.

_NPCScan can still detect them if they're spawned.

NPCScan Overlay (I believe?) is a nice addition to it, where you can see on your map where the routes and spawn areas of the rares are
