The rarest obtainable item in the game is the boe spellcaster leggings from zul'farrak or atleast they used to be not sure if they upped the drop rate. only a tiny amount (well under 100) have ever been recorded to have dropped.

I would have to agree, this is the rarest item. However, it's also unobtainable, so I would think there are two categories of rare items. Those that are no longer unavailable, and those that are still obtainable.
Talisman probably wins for unobtainable, as there is only one in game.
Others are old-school epic mount, before armor. The legendary caster staff from Naxx 1.0.
AQ Gate Opening mount is probably rarest still obtainable (forgive the poor grammar).
Hand of Ragnaros. The Legendary Bow from KT.
Rare and unobtainable is two very different things. What the most rare item is, I have no idea of, but just wanted to point out that the Necklace and Atiesh are not rare! They are rare to see (the necklace being impossible because the guy who had it quit during TBC), but they're not rare to get, because they're simply unobtainable.

Teebu's blazing longsword is ridiculously rare. It may not even drop any more. I know a majority of the ones that ever dropped were from the AQ events.
Pendelum of Doom was extremely rare and super valuable since it was a .01% drop off of a mob in Ulda. And there were only like 10 mobs that dropped it per instance. Very difficult to far. In BC, I know of people posting on forums looking, willing to transfer and spend large amount of gold to buy it for their twink.
Technically, the old epic mounts are feasibly still in game as a BoE. I doubt it, but the chance exists. Before the mounts went BoP, you could wrap them as a gift preserving the BoE status. There may be one on an old account still but this would probably be the most valuable item in game if it does exist wrapped still.
Bags with stats on them. Ie, Journeyman's backpack of fiery wrath was on Sen'jin for a little. I don't know what happened to it.
Reports of actual 372 trash items (with stats to match, not just 359 w/ wrong ilvl) dropping.
Discontinued items like blinding powder, thistle tea, old rogue poisons, etc...
That one hot dog recipie from scepter quest chain. (Is quest still in game?) while obtainable via quest, so few people have actually spent the time to do the quest chain let alone the resources. Any completionists must have for their cooking.
All of the "event gear" like from the original undead invasion before BC launched is unique and unobtainable.

I would only consider "rare" items to be BoE/Tradable. BoP, while it may be rare, is practically unobtainable as it would, 99% of the time, no longer be available thus the reason it is put in the rare category in the first place.
Talisman of Binding Shard was supposed to be part of the attunement for Molten Core hard mode, half a key for "extra" or additional bosses in a UBRS> BWL style progression, or probably (and most likely) the first version of the requirements for Thunderfury.
The design team that was in charge of MC, Strat, Naxx, and AQ 40 all left the game en masse before the release of WoW to join the studio that made Hellgate:London, so each one of those instances had to be essentially completed very very quickly. All of the unfinished ideas were discarded, and that included many things that did make it into Hellgate, as well as the idea of having a few legendaries per expansion, and an artifact, which I personally believe the Talisman turned into. As of this writing, even tho the quality "Artifact" is listed in the original game manual, none have made it to live.
The guys who ported over from EQ, as well as the other game companies Blizz absorbed, were light-years behind the crew that left, I'm afraid to say.
