Ghost Female Human Costume, low level quest item ? ( NOT the haloween one )

Hey there, im looking for an item that Transforms my char into a Human Femaly Ghost, i once saw one with it in a random Dungeon. i asked him what it was. and he said it was a reward from a low level Quest.

Does anyone know the Name of it, Or how to get it ? thanks in advance.

Are you sure it's a human ghostly transformation and not a night-elf.

One of the random forms it gives off is a human female with death knight eyes and a ghostly glow. Probably that.
