Probably Lunar: The Silver Star. Final Fantasy Six is close, and very near and dear to my heart, but Lunar engaged me just a little bit more.

Actually nvm, Modern Warfare 2's campaign hasn't been beaten for 4 years still
GTA - San Andreas. I really loved the story in that game.
Damn...that's a tough one. I've just played through so many damned games in 30 years I can't remember them all. Chrono Cross was definitely a goody, one of the few RPGs I've bothered to play through several times.
I agree with Woceip, Lunar was a damned good one too, though I didn't care for the second one near as much as the first. Hmm, and Star Ocean...man, there are just too many lol.
To the Moon - this game is just amazing. Funny though that it's a indie game.
God of War 3. The ending was extremely cathartic and fitting. It redeemed Kratos at the 11th hour but it didn't ignore all his wrongdoings. To me it was beautiful, even poetic, and it has forever cemented the God of War series in my mind as one of the best stories of all time.
The Witcher games both have a really engaging story. It's a great mix of good storytelling and fun gameplay. The Walking Dead is also the only game that ever made me shed a tear, so that says something about the story in itself.
Do Visual Novels count? If so, Saya no Uta.

Metal Gear Solid ... but i may be biased.
I went through MGS1 at least 5 times when i was a kid in my ps1, then played MGS2 and MGS3 (my fav) at release and finally finished the series with MGS4.
I basically grew up with Snake and his story.
Omikron - the nomad soul, back then I was like 15 years old and the game had a superb atmosphere. looking back at the screenshots, I still feel that it's excellent.
Dragon Age Origins. Simply amazing.
Planescape Torment, of course. And Warcraft 3 as well.

Tough question. But I would have to say Metal Gear Solid because I think it was the only game I've played that has made me cry. 4 specifically made me cry because all the previous games were just a build up to this climatic ending, just so perfect.
