Free refills? That doesn't sound logical. You pay for a drink, with free refills I would just put a big bucket under it and take it home.

There are plenty of restaurants that do free refills, and not all public toilets are pay on use either. Some places just don't want to sacrifice the profit by subsidizing such services.
Free refills? No wonder you guys over there are so.. uh.. big.
In the UK, some restaurants offer free re-fils - not all but some do, it's not as common as it is in the states.
As for the pay toilets... the fuck? Where did you find those o.O
Overall, the UK is a great place to visit - bar certain no-go areas, like certain part's of London or Cambridge. I'd say the state's needs to get socialized... (healthcare, relax you right wingers!)
Free Healthcare > Free Refills
It is difficult to free fools from the chains they revere.
Toby's Carvery in the UK has free refills (under Bottomless Glass). Pretty sure that there's others.

Most train stations have pay toilets. I know of no restaurant with a pay toilet.

Tipping? I dont understand the whole concept, people get paid for their job anyway.
Standard NHS charge* for prescriptions, remember. But it's a lot better than the American system.
*The worst I've ever had from the NHS was when I needed root canal from an accident I had 9 years earlier had caused one of my root nerves to die in my mouth. One dentist said they wanted just over £200 for a replaced tooth. I got a second opinion from another dentist and got it for £48. Both NHS.

also, ive only ever seen one pay for toilet in the UK, then again, i never use public toilets.
Like some posters said, some places do, and some don't. But the majority reason for it, is to make the puny tourists fall for our trap in giving us more money!

Erm... Democracy was founded in Greece (which, as you probably arn't aware, is in Europe).
I guess we don't have free refills because we care about our health.
Other odd things I've noticed? I don't have enough time or room on this thread to explain all the 'odd' things I've seen in the US.
Pay toilets????? I've seen probably about 4 in my whole life (which is a long time). I suggest you may need to explore a little more kid.
Oh, and Europe is a pretty large area. Believe it or not, there are thousands of different cultures in Europe.
Inb4 infraction from an overzealous US mod.
In Europe people are generally paid at least a liveable wage, therefore I only tip if the service is above ordinary. Being brought my food and drink is their job. It's probably a cultural thing, and certainly I do tip if I am in the states since that's what they do.
Pay toilets wise, I have only ever seen pay toilets at the train station, and that's normally to stop junkies going in and tying off. The public toilets in Edinburgh (and some buses) for instance have a blue light to prevent just that :P
You'll get free refills at your average US franchise but no, if I buy one drink I pretty much expect to get one drink.
