where and when to use this spell during leveling, in pvp and in pve ?

While leveling? Almost never.

It is used for cleaving.

Boss cleaves are about it, yeah. They have their own aggro table, and only 10% of your HP so if you sic them on a mob, it will turn and whack them in two hits. So aside from a boss and VERY situational use in pvp if you're attempting to confuse someone, it's pretty much limited to cleave/aoe fights on bosses.

In PvP it can be nice to throw one on a healer or any other target to keep them from drinking etc.
